What should an onboarding quiz actually cover?

Do new hires need an account to take it?

No. They open your link, enter a code or scan a QR code and start. A name is usually enough for onboarding, or require a verified email for a firmer record.

Can I use the same quiz for every intake?

Yes. Build the test once, then start a new run for each cohort, each with its own link, window and results, so January's starters never mix with June's.

How do I know who still needs to complete it?

Invite each intake from the participant directory and the status updates as people go: invited, started, completed. Reminders go to those who haven't finished, not the whole group.
